Thanks to Ms. Nolinske for creating a form for her 5th grade math class and providing me a copy to post on the web. The purpose of these forms is to help students to keep track of their study and test scores for each strand in each of the content areas (language arts & math) in the Leap 21 Achiever software. Most teachers set a study goal for the students to reach before allowing them to continue on to the test. For example, some teachers only allow students to take a test if they earn 80% or higher on the study portion of the program. |
